Pilot Ladder Inspection Requirements and Common Deficiencies
A vessel-focused guide to pilot ladder construction, rigging, inspection and documentation under SOLAS V/23, including defects that regularly make transfers unsafe.

Pilot transfer is a high-risk operation conducted at the interface between two moving vessels. SOLAS regulation V/23 requires arrangements that enable pilots to embark and disembark safely, and IMO recommendations address ladder construction, installation and use. Compliance depends on the entire transfer arrangement—not only the visible rope ladder.
Components to examine
Inspection should cover steps, spreaders, side ropes, seizings, chocks, retrieval arrangements, securing points, manropes where required, lighting, lifebuoy, communication, deck access, stanchions and any combination arrangement with an accommodation ladder. Winch reels and associated foundations require separate attention where fitted.
Common physical defects
- Cracked, split, painted or contaminated steps.
- Worn, glazed, cut, chemically damaged or inconsistently coloured side ropes.
- Loose seizings or steps that are no longer horizontal.
- Damaged or incorrectly spaced spreader steps.
- Unapproved repairs, knots or mechanical clamps.
- Missing identification and uncertain manufacturing history.
- Securing arrangements that load steps, chocks or weak deck fittings.
Rigging is part of compliance
A serviceable ladder can become unsafe when rigged over a discharge opening, against unsuitable hull projections or without a clean approach for the pilot boat. The ladder must rest firmly against the ship’s side, extend to the requested height above the water and provide safe access to the deck. The responsible officer should supervise rigging and remain in communication with the bridge.
Combination arrangements
Where the climb exceeds the SOLAS limit for a pilot ladder alone, an accommodation ladder is used with the pilot ladder. The lower platform should be horizontal and secured as required, and the pilot ladder must extend above the platform in the prescribed manner. Arrangement details and securing vary with ship construction; use approved plans and current IMO diagrams.
Inspection frequency and records
Crew should inspect the arrangement before use and maintain it under the planned-maintenance system. Periodic detailed inspection should consider age, storage, exposure and maker instructions. Record ladder identification, manufacture date, certificates, inspections, repairs and withdrawal. Never transfer identification from a rejected ladder to a replacement.
Storage and care
Store ladders clean, dry, ventilated and protected from sunlight, chemicals, paint, heat and deck traffic. Do not paint wooden steps or apply unapproved preservatives to ropes. Coil or stow the ladder without sharp bends and ensure emergency replacements are equally traceable.
Pre-transfer checklist
- Confirm the pilot’s requested boarding side and height.
- Inspect ladder and securing points in good light.
- Rig under a responsible officer’s supervision.
- Check illumination, lifebuoy, heaving line and communications.
- Verify deck access and handholds.
- Monitor freeboard, weather, speed and traffic throughout transfer.

Review the pilot ladder and transfer arrangements inspection, with major-port planning in Singapore and at Rotterdam. Submit photographs, ladder certificates and schedule through the quotation form.
